Cocker Spaniel behavior outside the house

My cocker spaniel is a wonderful sweet dog at home. However, when I take her for walks or to the park she becomes very frightened, nervous and hyper-active. She either barks at people/dogs or tries to jump up at them.

This has has also become a problem when I take to to be groomed .

This breaks my heart because I would really like to make her as much a part of my life as possible. However this behavior makes it difficult for me to include her in activities outside my house.
