Sounds like maybe a congential issue (the CKD, that is)?

The most important thing is that she eat, and prescription kidney food often becomes unappealing to cats because it's so low in protein. I would suggest to your friend to start offering her kitty a buffet of different foods (wet food especially!) and not to worry about the special diet (which is controversial at best - newer thinking is that it isn't about the amount of protein, but the quality).
